This form replaces the traditional paper application for the following resident and fellow leadership positions appointed by the AMA Board of Trustees:

• ACGME Board of Directors – Resident member
• AMA Foundation Board of Directors – Resident and Fellow Section representative
• Council on Ethical & Judicial Affairs- Resident member
• Council on Legislation – Resident member
• Council on Long Range Planning and Development – Resident member
• Surgical Caucus Executive Committee- Resident member

Note: If you wish to apply for more than one position, you must complete this application separately for each position.
